The staff at Church Street Surgery would like to express our sincere condolences on your recent loss. We know that this is a very difficult time. We would like to provide you with some useful information about the registration process following the death of a loved one at home.
Firstly, the Medical Examiner Office will be notified by the responsible GP or the GP that treated them in their last illness. The medical examiner will review their medical notes and come to conclusion as to the cause of death. The surgery will then be informed of this, and the Medical Certificate of Death is completed by the GP. This is then emailed to the Registrar’s Office at Bishop’s Stortford Registers Office at Riverside House, 2 Hockerill Street, Bishop’s Stortford, Herts CM23 2DL
Afterwards you can then contact them on 03001234045 or “register-a-death” to register the death.
In some cases when the cause of death is not known, this would be discussed with the coroner who will decide if an autopsy is needed. If this is the case, you will be kept informed.
The cremation form if needed would be done by the GP and sent directly to the funeral home of your choice.

- “Tell Us Once” – This a government website that gives more information about registering a death, what documents you will need. There is also an online portal that enables you notify government agencies about the death e.g., HMRC, Passport Office, DWP, DVLA, the local council
