Young Persons Social Prescribing Service

Stort Valley Healthcare have evolved Social Prescribing to Young People between 11 to 25 years old, the first of it’s kind in the country. Social Prescribing intends to develop Stronger Partnerships, between patient, their community and supporting services within Stort Valley and Villages locality and beyond.

If you are patient, aged between 11 to 25 years old and feel you might benefit from the service, please make an appointment to see the GP who can assess referring you into the service.
