Access to online services connected to your medical record is now via SystmOnline.
To register you will need to come into the surgery with photo ID.
Once registered, you will be able to use your online account to:
- book routine GP appointments
- cancel appointments (including nurse or HCA appointments)
- order repeat prescriptions
- view problem codes and test results
- view current medication, allergies and your NHS number and
- update your contact details.
Following changes to the GP contract, practices across the country are required to make more of a patients' medical record available to that patient online, if they wish to access the information. Due to these changes the process for registering for online services has become more stringent. Each patient wishing to register for online services must first read the information leaflet, also available from the surgery, and complete a registration form. You must come into the surgery, with photo ID and proof of address to complete the registration process. Acceptable forms of ID include a passport or EU driving licence.
Each person must register for themselves. Parents (or those with parental responsibility) of children under 13 years of age can register for proxy access to online services for their child. We will need to see the photo ID of the parent and the birth certificate for each child. Once the child reaches 13 year of age, access to online services will be suspended. It can be reactivated if the child gives their consent to their parent having access. This must be done in person, with ID. Please note that parents do not have an automatic right to access the records of their children who are aged 13, 14 or 15: the consent of the child must be obtained.
At 16 years of age online access accounts will be suspended and only the patient may re-register.
NB: Not all of the services within SystmOnline are currently available at Church Street Partnership, and this is clearly indicated in the online service request area. We do not currently offer Full Clinical Record. Questionnaires will be made available to patients with some long-term health problems to help manage their condition: we will be trialling the asthma questionnaire from January 2018. Please do not complete questionnaires relating to medical conditions that you do not have.

We hope that patients will find the online services useful. However, if an individual abuses their online account we will block the access and that patient will need to use the other means of booking appointments, requesting repeat prescriptions etc.
Inappropriate use includes, but is not restricted to:
- sending abusive messages
- booking, then wasting appointments
- over-ordering medication
- repeatedly requesting services that are not available.
