COVID-19 Vaccinations – Information

COVID Vaccine Exemptions

Patients wishing to apply for a COVID vaccine medical exemption certificate should contact 119 to request a form. You can post or bring the form to the surgery where this will be reviewed by a GP. You may be contacted to discuss in further detail. We are unable to guarrantee that you will be eligible and this is discussed on a case by case basis.

Face Coverings

It remains mandatory to wear a face covering when you visit a hopsital, doctors surgery and on public transport.

GP’s are unable to provide individual risk assessments or letters for patients who wish to be exempt from wearing a face covering.

Patients seeking medical advice about returning to work or school

Patients who are in the vulnerable (but not shielded) group who have concerns about returning to work may contact the surgery for information or advice to share with their employers. While GP’s may decide to provide a medical summary for patients, your GP’s will not be able to give advice about returining to work, as they will not know the individual circumstances of a patient’s workplace. This is a HR issue between the patient and their employer. GP’s will also be unable to provide letters for parents who are concerned about their children returning to school. It is for parents to determine if it is safe for their children, in discussion with the school whether to return to school.
