Confidentiality

All staff are bound by strict rules of confidentiality. We cannot give out any information to third parties (including spouses and parents of children aged 16 years or over) without the consent of the patient. This includes disclosing if a patient is on the premises.

We are a computerised practice and registered under the Data Protection Act and with the Information Commissioner’s Office.

Freedom Of Information– Publication Scheme

The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available.

Information is available on application to the practice manager.

Fair Processing Notice

To make sure you receive the most appropriate care and treatment, it may be necessary to share the information in your health records with other health professionals and organisations that are involved in your care. Computer records are now set up in such a way that health professionals from one organisation may be able to look at your records from another – for example, the hospital may be able to look at your GP record. They must ask your permission before they do this, and you have the right to refuse. Anonymised data is used by the NHS for purposes such as service planning. If you wish to opt out please download and complete the form below and return it to the surgery. We will then add a code that prevents extraction of your anonymised data.

Information Sharing

NHS Digital uses health and care information for research and to help plan services. Previously, if you had wished to opt out of your information from being used for purposes other than you direct care, you notified your GP surgery and a code was added to your record which acted as a block.

From May 2018 your GP surgery can no longer record your preference to this form of data sharing. Please read the leaflet below for further information
